Change Address in Aadhaar: Using HoF Consent and the Gazetted Officer Form
To change the address in Aadhaar, you normally need an address proof (POA) in your own name. But many people, such as children, wives or parents, do not have such a document in their own name. For them, UIDAI offers two easy routes: an online address update with the consent of the Head of Family (HoF), or an address certificate in the UIDAI format issued by an authorised person such as a Gazetted Officer. Both methods are explained here in simple language.
Key Features of Both Methods
- The HoF method needs no address proof in the applicant's name. The address in the Head of Family's Aadhaar is used.
- Any Aadhaar holder above 18 years can be an HoF. You must provide a proof of relationship or, if you do not have one, a self-declaration by the HoF.
- The HoF-based update is done online on the myAadhaar portal, and UIDAI has set a fee of ₹75 for it.
- The HoF must log in to the portal and give consent within 30 days of getting the request notification.
- The Gazetted officer method helps when you have no address proof and the HoF option is not possible.
- In both methods your Aadhaar-linked mobile number must be active, because confirmation is done through OTP.
Method 1: Change Address Online with HoF (Head of Family) Consent
- Open myaadhaar.uidai.gov.in and log in with your Aadhaar number and OTP.
- Choose the Update Address option and select "Head of Family (HoF) based address update".
- Enter the HoF's Aadhaar number. To protect the HoF's privacy, no other details are shown on screen.
- Upload the Proof of Relationship. If it is not available, upload the HoF's Self Declaration in the UIDAI format.
- Review the details, submit and pay the ₹75 fee online. You will receive a Service Request Number (SRN).
- An SMS goes to the HoF's mobile. The HoF must log in to the portal and approve the request with an OTP within 30 days.
- Keep tracking the status with your SRN. After approval the address is updated and you can download your new Aadhaar.
Proof of Relationship and Self-Declaration for HoF
- A proof of relationship should be a document that shows the names of both the applicant and the HoF along with their relationship, such as a ration card, birth certificate, marriage certificate or passport.
- A self-declaration usually contains the HoF's name and Aadhaar number, the applicant's name and Aadhaar number, their relationship, the new address, and the HoF's consent and signature.
- The correct self-declaration format is provided on the UIDAI portal during this process. Always use that format.
Method 2: Change Address with a Gazetted Officer Certificate
- Download the UIDAI-prescribed "Address Certificate" format from the UIDAI website or collect it from an Aadhaar center.
- Fill in your name, Aadhaar number and new address and attach a recent passport-size photo.
- Get the form verified by an authorised person from your area, such as a Gazetted Officer, Tehsildar, MP/MLA/Councillor or Gram Panchayat head (Sarpanch). They will add their name, designation, stamp, signature and date.
- Visit your nearest Aadhaar center with your original Aadhaar and the completed certificate. Booking an appointment in advance saves you from crowds.
- Submit the address update request at the center, complete identity verification, pay the prescribed fee and collect the acknowledgement slip with the Update Request Number (URN).
- Track the update status with your URN and download the new Aadhaar once it is updated.
What to Carry to the Aadhaar Center
- Your original Aadhaar card or Aadhaar number, and your active Aadhaar-linked mobile number.
- The address certificate in the UIDAI format, filled in, with your photo and verified by the authorised officer.
- A valid identity proof and your appointment reference number / slip (if you booked one).
Important Points
- If the HoF rejects the request or does not respond within 30 days, the request is closed and the fee paid is not refunded. Talk to the HoF before you apply.
- An address certificate is valid only for a limited period (usually a few months from the date of issue), so submit it soon after getting it.
- Fill in only correct information. Giving or certifying false details can lead to legal action.
- Pay only the fee prescribed by UIDAI. Do not pay extra money to any agent or middleman.
- Type the address exactly as it appears on the uploaded document or form, so that the request is not rejected.

- 18 वर्ष से अधिक उम्र का कोई भी आधार धारक HoF बन सकता है। रिश्ते का प्रमाण, या न हो तो HoF का स्वघोषणा पत्र, देना होता है।
- HoF आधारित पता अपडेट myAadhaar पोर्टल पर ऑनलाइन होता है और इसके लिए UIDAI ने ₹75 शुल्क तय किया है।
- HoF को अनुरोध की सूचना मिलने के बाद 30 दिन के भीतर पोर्टल पर लॉगिन करके सहमति देनी होती है।
- गजटेड अधिकारी वाला तरीका तब काम आता है जब आपके पास न पता प्रमाण है और न ही HoF वाला विकल्प संभव है।

तरीका 1: HoF (परिवार के मुखिया) की सहमति से ऑनलाइन पता बदलें
- myaadhaar.uidai.gov.in खोलें और अपने आधार नंबर व OTP से लॉगिन करें।
- पता अपडेट (Update Address) का विकल्प चुनें और "Head of Family (HoF) based address update" चुनें।
- HoF का आधार नंबर डालें। HoF की गोपनीयता के लिए उनकी अन्य जानकारी स्क्रीन पर नहीं दिखती।
- रिश्ते का प्रमाण (Proof of Relationship) अपलोड करें। अगर वह उपलब्ध न हो, तो UIDAI के फॉर्मेट में HoF का स्वघोषणा पत्र (Self Declaration) अपलोड करें।
- जानकारी जांचकर सबमिट करें और ₹75 शुल्क ऑनलाइन भरें। आपको एक सर्विस रिक्वेस्ट नंबर (SRN) मिलेगा।

- SRN से स्टेटस देखते रहें। मंज़ूरी के बाद पता अपडेट हो जाएगा और आप नया आधार डाउनलोड कर सकेंगे।
HoF के लिए रिश्ते का प्रमाण और स्वघोषणा पत्र
- रिश्ते का प्रमाण ऐसा दस्तावेज़ हो जिसमें आवेदक और HoF दोनों के नाम व रिश्ता दर्ज हो, जैसे राशन कार्ड, जन्म प्रमाणपत्र, विवाह प्रमाणपत्र या पासपोर्ट।
- स्वघोषणा पत्र में आमतौर पर HoF का नाम व आधार नंबर, आवेदक का नाम व आधार नंबर, आपसी रिश्ता, नया पता और HoF की सहमति व हस्ताक्षर होते हैं।
- स्वघोषणा पत्र का सही फॉर्मेट UIDAI पोर्टल पर इसी प्रक्रिया के दौरान मिलता है। हमेशा वही फॉर्मेट इस्तेमाल करें।
तरीका 2: राजपत्रित अधिकारी (Gazetted) के प्रमाणपत्र से पता बदलें
- UIDAI का तय फॉर्मेट "Address Certificate / पते का प्रमाणपत्र" UIDAI की वेबसाइट से डाउनलोड करें या आधार केंद्र से लें।
- फॉर्म में अपना नाम, आधार नंबर और नया पता भरें और अपनी ताज़ा पासपोर्ट साइज़ फोटो लगाएं।
- अपने क्षेत्र के अधिकृत अधिकारी, जैसे राजपत्रित अधिकारी, तहसीलदार, सांसद/विधायक/पार्षद या ग्राम पंचायत प्रमुख (सरपंच) से फॉर्म पर सत्यापन कराएं। वे अपना नाम, पदनाम, मुहर, हस्ताक्षर और तारीख लगाएंगे।
- मूल आधार और भरा हुआ प्रमाणपत्र लेकर नज़दीकी आधार केंद्र जाएं। पहले से अपॉइंटमेंट बुक कर लें तो भीड़ से बचत होगी।
- केंद्र पर पता अपडेट का अनुरोध दें, पहचान की पुष्टि कराएं और निर्धारित शुल्क देकर अपडेट रिक्वेस्ट नंबर (URN) वाली पर्ची लें।
- URN से आधार अपडेट का स्टेटस जांचते रहें और अपडेट होने पर नया आधार डाउनलोड करें।
आधार केंद्र पर क्या साथ ले जाएं?
- आपका मूल आधार कार्ड या आधार नंबर, और आधार से जुड़ा चालू मोबाइल नंबर।
- UIDAI के फॉर्मेट में भरा हुआ, फोटो लगा और अधिकृत अधिकारी द्वारा सत्यापित पते का प्रमाणपत्र।
- पहचान का कोई मान्य प्रमाण और अपॉइंटमेंट का रेफरेंस नंबर / स्लिप (अगर बुक किया हो)।
ज़रूरी बातें
- अगर HoF अनुरोध अस्वीकार कर दे या 30 दिन में कोई जवाब न दे, तो अनुरोध बंद हो जाता है और जमा किया गया शुल्क वापस नहीं होता। इसलिए भरने से पहले HoF से बात कर लें।
- पते का प्रमाणपत्र सीमित अवधि के लिए मान्य होता है (आमतौर पर जारी होने की तारीख से कुछ महीने)। इसलिए बनवाते ही जमा कराएं।
- फॉर्म में केवल सही जानकारी भरें। गलत जानकारी देने या प्रमाणित करने पर कानूनी कार्रवाई हो सकती है।
- आधार सेवाओं के लिए केवल UIDAI द्वारा निर्धारित शुल्क ही दें। किसी एजेंट या दलाल को अतिरिक्त पैसे न दें।
- पते की स्पेलिंग अपलोड किए गए दस्तावेज़ या फॉर्म के अनुसार ही डालें, ताकि अनुरोध रिजेक्ट न हो।
