Date of Birth (DOB) Correction in Aadhaar: Annexure / Self-Declaration Forms and Process for All Ages
A wrong date of birth in Aadhaar can cause trouble in school admissions, passports, pensions, banking and government schemes. UIDAI allows DOB correction only at an Aadhaar center, and the documents required depend on your age. In some cases you must also submit an affidavit (Annexure) or a self-declaration. This guide explains the process and forms for every age group, from children to senior citizens, in simple language.
Key Features
- DOB correction cannot be done online. It is done at an Aadhaar Seva Kendra with biometric verification. Online, only appointment booking and status tracking are available.
- Under the normal process, the date of birth can be changed only once in a lifetime.
- For children aged 0 to 5 and 5 to 18 years, only a birth certificate is accepted for DOB correction.
- People above 18 can correct the DOB with a valid proof of date of birth from the UIDAI list (such as a birth certificate, passport or education marksheet).
- In some cases an affidavit (Annexure-I / IA) or a self-declaration (Annexure-II) is required. The sections below explain which one applies when.
- A fee prescribed by UIDAI applies for DOB correction (currently ₹75).
Documents Required by Age Group
- 0 to 5 years: The child's birth certificate and a parent's Aadhaar. The parent must accompany the child.
- 5 to 18 years: The child's birth certificate, the child's Aadhaar, and the Aadhaar of a parent or guardian.
- Above 18 years: Any valid proof of date of birth from the UIDAI list, such as a birth certificate, valid Indian passport, Class 10/12 marksheet or certificate from a recognised institution, or a government service / pension photo ID card.
- NRI below 18 years: A birth certificate or an Indian passport.
Forms and Annexures: Which One Applies When?
- Aadhaar Update/Correction Form: Required for every correction. Enter your Aadhaar number and fill in only the field you want corrected (date in DDMMYYYY format). Children have a separate age-specific form, available at the center or for download from the UIDAI website.
- Annexure-I (for adults) and Annexure-IA (for children) affidavit: Needed when the date of birth is already recorded as verified on the basis of a document such as a birth certificate and a further correction is required. The center confirms whether your case falls here, as per UIDAI guidelines.
- Annexure-II self-declaration: Needed when the date in Aadhaar differs from the document you submitted last time (because of an operator error) and that document is no longer valid or you no longer hold it.
- An affidavit usually has to be prepared on stamp paper / before a notary. Confirm the exact method with your Aadhaar center. Always take the formats from the UIDAI website or the center only.
DOB Correction Process (Steps)
- Keep a valid proof of the correct date of birth ready, and check that the name on it matches the name in your Aadhaar.
- Book a slot at your nearest Aadhaar center through "Book an Appointment" on myaadhaar.uidai.gov.in.
- Fill in the update/correction form at the center. Fill only the date of birth field.
- Submit your original documents and, if required, the Annexure. For a child, a parent/guardian must come along.
- Complete biometric verification and pay the prescribed fee.
- Collect the slip with the Update Request Number (URN), keep tracking the status and download the new Aadhaar once updated.
Important Points
- The date of birth can normally be changed only once, so apply the first time with the correct date and the right document.
- The date entered in Aadhaar must exactly match the date on your document, otherwise the request may be rejected.
- Using fake documents or wrongly changing an age can lead to legal action.
- If the birth certificate itself has a mistake, first get it corrected by the Registrar of Births and Deaths, then apply for the Aadhaar correction.
- Pay only the fee prescribed by UIDAI. Do not pay extra money to any agent or middleman.

- जन्मतिथि का सुधार ऑनलाइन नहीं होता। यह आधार सेवा केंद्र पर बायोमेट्रिक पुष्टि के साथ होता है। ऑनलाइन सिर्फ अपॉइंटमेंट बुकिंग और स्टेटस ट्रैकिंग की सुविधा है।
- सामान्य प्रक्रिया में जन्मतिथि पूरे जीवन में केवल एक बार बदली जा सकती है।
- 0 से 5 वर्ष और 5 से 18 वर्ष के बच्चों के लिए जन्मतिथि सुधार में केवल जन्म प्रमाणपत्र मान्य है।
- 18 वर्ष से ऊपर के लोग UIDAI की सूची में दिए मान्य जन्मतिथि प्रमाण (जैसे जन्म प्रमाणपत्र, पासपोर्ट, शैक्षणिक अंकपत्र) से सुधार करा सकते हैं।

उम्र के अनुसार ज़रूरी दस्तावेज़
- 0 से 5 वर्ष: बच्चे का जन्म प्रमाणपत्र और माता/पिता का आधार। माता-पिता को साथ जाना होगा।
- 5 से 18 वर्ष: बच्चे का जन्म प्रमाणपत्र, बच्चे का आधार और माता/पिता या अभिभावक का आधार।
- 18 वर्ष से ऊपर: UIDAI की सूची का कोई मान्य जन्मतिथि प्रमाण, जैसे जन्म प्रमाणपत्र, वैध भारतीय पासपोर्ट, मान्यता प्राप्त संस्था की 10वीं/12वीं की मार्कशीट या प्रमाणपत्र, सरकारी सेवा या पेंशन का फोटो पहचान पत्र।
- 18 वर्ष से कम NRI: जन्म प्रमाणपत्र या भारतीय पासपोर्ट।
फॉर्म और Annexure: कौन-सा कब लगता है?
- आधार अपडेट/करेक्शन फॉर्म: हर सुधार में ज़रूरी। इसमें अपना आधार नंबर और केवल वही खाना भरें जिसमें सुधार चाहिए (तारीख DDMMYYYY में)। बच्चों के लिए उम्र के अनुसार अलग फॉर्म होता है, जो केंद्र पर मिलता है या UIDAI वेबसाइट से डाउनलोड होता है।
- Annexure-I (वयस्कों के लिए) और Annexure-IA (बच्चों के लिए) शपथ-पत्र: जब जन्मतिथि पहले से जन्म प्रमाणपत्र जैसे दस्तावेज़ के आधार पर दर्ज (verified) हो और दोबारा सुधार की ज़रूरत पड़े, तब लगता है। कौन-सा मामला इसमें आता है, यह केंद्र पर UIDAI की गाइडलाइन से तय होता है।
- Annexure-II स्वघोषणा पत्र: जब आधार में दर्ज तारीख आपके पिछली बार जमा किए दस्तावेज़ से अलग है (ऑपरेटर की गलती से) और वह दस्तावेज़ अब मान्य नहीं या आपके पास नहीं है।
- शपथ-पत्र आमतौर पर स्टाम्प पेपर/नोटरी से बनवाना होता है। सही तरीका अपने आधार केंद्र से पूछ लें। सभी फॉर्मेट UIDAI की वेबसाइट या केंद्र से ही लें।
जन्मतिथि सुधार की प्रक्रिया (स्टेप्स)
- सही जन्मतिथि का मान्य दस्तावेज़ तैयार रखें और जाँच लें कि उसमें नाम आपके आधार के नाम से मेल खाता है।
- myaadhaar.uidai.gov.in से "Book an Appointment" करके नज़दीकी आधार केंद्र पर स्लॉट बुक करें।
- केंद्र पर अपडेट/करेक्शन फॉर्म भरें। सिर्फ जन्मतिथि वाला खाना भरें।
- मूल दस्तावेज़ और (ज़रूरत हो तो) Annexure के साथ जमा करें। बच्चे के मामले में माता-पिता/अभिभावक साथ जाएं।
- बायोमेट्रिक पुष्टि कराएं और निर्धारित शुल्क दें।
- अपडेट रिक्वेस्ट नंबर (URN) वाली पर्ची लें, स्टेटस देखते रहें और अपडेट होने पर नया आधार डाउनलोड करें।
ज़रूरी बातें
- जन्मतिथि सामान्यतः एक ही बार बदलती है, इसलिए पहली बार में ही सही तारीख और सही दस्तावेज़ के साथ आवेदन करें।
- आधार में भरी जाने वाली तारीख दस्तावेज़ की तारीख से बिल्कुल मेल खानी चाहिए, वरना अनुरोध रिजेक्ट हो सकता है।
- फर्जी दस्तावेज़ या गलत तरीके से उम्र बदलवाने पर कानूनी कार्रवाई हो सकती है।
- जन्म प्रमाणपत्र में ही गलती हो तो पहले जन्म-मृत्यु पंजीयक से उसे ठीक करवाएं, फिर आधार में सुधार कराएं।
- आधार सेवाओं के लिए केवल UIDAI द्वारा निर्धारित शुल्क ही दें। किसी एजेंट या दलाल को अतिरिक्त पैसे न दें।
