Passport for a Child with a Single Parent or Without Parents: Process and Documents
Passport rules allow a child to get a passport even when only one parent is raising the child, or when there are no living parents. The application form needs the name of only one parent or legal guardian, and the passport office can print just one name if you ask. This guide covers a single mother or father, divorced or separated parents, a widowed parent, a child born outside marriage, orphaned children and children under a legal guardian.
Key Features
- The online form asks for the name of the father or mother or legal guardian, not both. You can also request that only one name is printed.
- A single parent with custody can apply without the other parent's signature by giving Annexure C (declaration where one parent has not consented).
- Divorced parents need the divorce decree or court order showing custody. Only the parent with custody applies.
- For a child born outside marriage, Annexure D signed by both biological parents (with the Appendix 12 affidavit) is used when both names are to be entered.
- An orphaned child with no date of birth proof can submit a declaration from the Head of the Orphanage or Child Care Home on its letterhead.
- A legal guardian signs the declaration in place of the parents and applies on the child's behalf.

Exact requirements depend on your case. The Document Advisor on the Passport Seva portal shows the current list after you choose the applicant category.
Process
- Log in at passportindia.gov.in and select Apply for Fresh Passport (Re-issue if the child already has a passport).
- In the family section, fill the name of the one parent or legal guardian. If you want only one name printed, choose that option on the form.
- Keep the right declaration and supporting papers ready (see the table above), then submit the form and pay the fee online.
- Book an appointment at the nearest PSK or POPSK. The applying parent or guardian must bring the child.
- At the PSK, show the originals with self-attested copies. The officer may ask a few questions about custody or guardianship.
- After verification the passport is sent by Speed Post. Track it using your file number.
Documents to Carry
- Child's date of birth proof: birth certificate (preferred), or another listed document. For orphans, the Head of Orphanage declaration.
- Proof of address in the name of the applying parent or guardian.
- ID and original passport of the applying parent or guardian, if they have a passport.
- Annexure C or D (or the guardian's declaration) as per your case.
- Court orders, divorce decree, death certificate, adoption deed or guardianship order, wherever applicable.
- Application receipt and appointment letter.
Important Points
- Where a court has given a specific order about custody or travel, the passport office follows that order. If both parents are in court proceedings, court permission or both parents' consent may be needed.
- Annexures are plain-paper declarations. You do not need a notary or stamp paper.
- Give true facts about custody and parentage. A false declaration can lead to rejection and legal action.
- Name, date of birth and parents' names must match across the birth certificate, Aadhaar and school records.
- Rules can change. Check passportindia.gov.in or call 1800-258-1800 for the latest information.

सिंगल पेरेंट या बिना माता-पिता के बच्चे का पासपोर्ट: प्रक्रिया और दस्तावेज़
पासपोर्ट नियमों के तहत ऐसे बच्चे का भी पासपोर्ट बन सकता है जिसे सिर्फ एक अभिभावक पाल रहा हो या जिसके माता-पिता न हों। आवेदन फॉर्म में सिर्फ एक अभिभावक या कानूनी अभिभावक का नाम देना होता है, और चाहें तो पासपोर्ट पर केवल एक ही नाम छपवाया जा सकता है। यहां सिंगल मदर या फादर, तलाकशुदा या अलग रह रहे माता-पिता, विधवा/विधुर अभिभावक, विवाह के बाहर जन्मे बच्चे, अनाथ बच्चे और कानूनी अभिभावक वाले बच्चों के लिए प्रक्रिया बताई गई है।
मुख्य जानकारी (Key Features)
- ऑनलाइन फॉर्म में पिता या माता या कानूनी अभिभावक में से किसी एक का नाम मांगा जाता है, दोनों का नहीं। चाहें तो पासपोर्ट पर सिर्फ एक नाम छपवा सकते हैं।
- कस्टडी वाला सिंगल पेरेंट दूसरे अभिभावक के हस्ताक्षर के बिना Annexure C (एक अभिभावक की सहमति न मिलने की घोषणा) देकर आवेदन कर सकता है।
- तलाकशुदा अभिभावक को तलाक का आदेश या कस्टडी दिखाने वाला कोर्ट ऑर्डर चाहिए। आवेदन वही अभिभावक करता है जिसके पास कस्टडी है।
- विवाह के बाहर जन्मे बच्चे के मामले में दोनों नाम दर्ज कराने के लिए दोनों जैविक माता-पिता के हस्ताक्षर वाला Annexure D (और Appendix 12 शपथ-पत्र) लगता है।
- जन्मतिथि का प्रमाण न रखने वाला अनाथ बच्चा अनाथालय या चाइल्ड केयर होम के प्रमुख की लेटरहेड पर घोषणा दे सकता है।
- कानूनी अभिभावक माता-पिता की जगह घोषणा पर हस्ताक्षर करता है और बच्चे की ओर से आवेदन करता है।

सही दस्तावेज़ आपके मामले पर निर्भर करते हैं। आवेदक की श्रेणी चुनने के बाद पासपोर्ट सेवा पोर्टल का Document Advisor ताज़ा सूची दिखाता है।
प्रक्रिया
- passportindia.gov.in पर लॉगिन करें और Apply for Fresh Passport चुनें (बच्चे का पासपोर्ट पहले से है तो Re-issue)।
- परिवार वाले हिस्से में किसी एक अभिभावक या कानूनी अभिभावक का नाम भरें। सिर्फ एक नाम छपवाना हो तो फॉर्म में वही विकल्प चुनें।
- अपने मामले की सही घोषणा और सहायक कागज़ तैयार रखें (ऊपर की तालिका देखें), फिर फॉर्म सबमिट करें और फीस ऑनलाइन जमा करें।
- नज़दीकी PSK या POPSK में अपॉइंटमेंट बुक करें। आवेदन करने वाले अभिभावक या संरक्षक को बच्चे को साथ लाना होगा।
- PSK पर मूल दस्तावेज़ और स्व-सत्यापित कॉपी दिखाएं। अधिकारी कस्टडी या संरक्षकता के बारे में कुछ सवाल पूछ सकते हैं।
- जांच के बाद पासपोर्ट स्पीड पोस्ट से आता है। फाइल नंबर से स्टेटस देखें।
साथ ले जाने वाले दस्तावेज़
- बच्चे की जन्मतिथि का प्रमाण: जन्म प्रमाणपत्र (बेहतर), या सूची का अन्य दस्तावेज़। अनाथ बच्चे के लिए अनाथालय प्रमुख की घोषणा।
- आवेदन करने वाले अभिभावक या संरक्षक के नाम का पते का प्रमाण।
- आवेदक अभिभावक या संरक्षक का पहचान पत्र और (अगर है तो) मूल पासपोर्ट।
- आपके मामले के अनुसार Annexure C या D (या संरक्षक की घोषणा)।
- जहां लागू हो वहां कोर्ट के आदेश, तलाक का आदेश, मृत्यु प्रमाणपत्र, गोदनामा या संरक्षकता का आदेश।
- एप्लीकेशन रसीद और अपॉइंटमेंट लेटर।
ज़रूरी बातें
- अगर अदालत ने कस्टडी या यात्रा पर कोई खास आदेश दिया है तो पासपोर्ट कार्यालय उसी आदेश को मानता है। दोनों अभिभावकों के बीच कोर्ट केस चल रहा हो तो अदालत की अनुमति या दोनों की सहमति चाहिए हो सकती है।
- Annexure सादे कागज़ की घोषणाएं हैं। इनके लिए नोटरी या स्टांप पेपर नहीं चाहिए।
- कस्टडी और माता-पिता के बारे में सही तथ्य ही दें। गलत घोषणा पर आवेदन रद्द हो सकता है और कानूनी कार्रवाई भी हो सकती है।
- नाम, जन्मतिथि और माता-पिता के नाम जन्म प्रमाणपत्र, आधार और स्कूल रिकॉर्ड में एक जैसे होने चाहिए।
