Passport for Married Women: New Passport, Surname Change and Spouse Name
A married woman can apply for a new passport, or update her existing passport after marriage, on the Passport Seva portal. A new passport is a fresh application. Changing the surname or adding the husband's name on an existing passport is done through Re-issue with a marriage certificate, or the Joint Photo Declaration (Annexure J) signed by both spouses. This guide explains each situation with the documents needed.
Key Features
- A marriage certificate is not compulsory for applying for a fresh passport. The old requirement of Annexure K was removed.
- To change surname or add spouse name on an existing passport, apply for Re-issue and submit a marriage certificate or Annexure J.
- Annexure J is a joint photo declaration on plain paper, signed by both husband and wife. No notary or stamp paper is needed.
- After divorce, surname can be changed back or the spouse's name deleted with the divorce decree.
- After re-marriage, submit the divorce decree or first spouse's death certificate plus the new marriage proof.
- The fee is the normal fee for fresh or re-issue passport. There is no separate fee for the name change itself.

Process for a Married Woman
- Log in at passportindia.gov.in. Choose Apply for Fresh Passport if you have no passport, or Re-issue if you want to change the name or add the spouse's name.
- For re-issue, select the reason Change in existing personal particulars and tick surname or spouse name. Enter the new name exactly as in your updated Aadhaar or PAN.
- Fill the form, pay the fee online and book an appointment at a PSK. Tatkal can be chosen if you are eligible.
- If you do not have a marriage certificate, prepare Annexure J: a plain paper declaration with a joint photograph, signed by both husband and wife.
- Visit the PSK with the old passport, the marriage proof, ID and address proof, and self-attested copies. Where available, bring the spouse's passport copy.
- After verification, the new passport arrives by Speed Post with the updated name or spouse details.
Documents to Carry
- Old passport (original) and self-attested copies of its first and last pages, for a re-issue.
- Marriage certificate or Annexure J (joint photo declaration signed by both spouses).
- Date of birth proof and photo ID (Aadhaar, PAN, voter ID and so on).
- Address proof in your name, or spouse's passport copy where the address is the same.
- Divorce decree or death certificate of earlier spouse, if applicable.
- Application receipt and appointment letter.
Important Points
- Update your name in Aadhaar and PAN first, or at least keep your documents consistent, so there is no mismatch with the passport.
- Name changes on the passport do not change the passport number, but the new booklet replaces the old one, which is returned cancelled.
- Visa and flight tickets must match the name printed on your passport. If you plan to travel soon, update the passport well in advance.
- A name change that is only a spelling correction may need fewer documents than a new surname.

मुख्य जानकारी (Key Features)
- नए पासपोर्ट के आवेदन के लिए विवाह प्रमाणपत्र अनिवार्य नहीं है। पुरानी Annexure K की शर्त हटा दी गई है।
- मौजूदा पासपोर्ट में सरनेम बदलने या पति का नाम जोड़ने के लिए Re-issue करें और विवाह प्रमाणपत्र या Annexure J लगाएं।
- Annexure J सादे कागज़ पर पति-पत्नी दोनों के हस्ताक्षर वाली संयुक्त फोटो घोषणा है। नोटरी या स्टांप पेपर नहीं चाहिए।
- तलाक के बाद तलाक के आदेश के साथ सरनेम वापस बदला जा सकता है या जीवनसाथी का नाम हटवाया जा सकता है।
- दोबारा शादी होने पर तलाक का आदेश या पहले जीवनसाथी का मृत्यु प्रमाणपत्र और नई शादी का प्रमाण देना होता है।
- फीस नए या री-इश्यू पासपोर्ट की सामान्य फीस ही है। नाम बदलने की अलग से फीस नहीं लगती।

- passportindia.gov.in पर लॉगिन करें। पासपोर्ट नहीं है तो Apply for Fresh Passport और नाम बदलवाना या पति का नाम जोड़ना हो तो Re-issue चुनें।
- री-इश्यू में कारण Change in existing personal particulars चुनें और सरनेम या जीवनसाथी का नाम टिक करें। नया नाम अपने अपडेटेड आधार या पैन के अनुसार ही लिखें।
- फॉर्म भरें, फीस ऑनलाइन जमा करें और PSK में अपॉइंटमेंट बुक करें। पात्र हों तो तत्काल चुन सकती हैं।
- विवाह प्रमाणपत्र न हो तो Annexure J तैयार करें: सादे कागज़ पर संयुक्त फोटो के साथ घोषणा, जिस पर पति-पत्नी दोनों के हस्ताक्षर हों।
- पुराना पासपोर्ट, विवाह का प्रमाण, पहचान और पते का प्रमाण और स्व-सत्यापित कॉपी लेकर PSK जाएं। हो सके तो जीवनसाथी के पासपोर्ट की कॉपी भी रखें।
- जांच के बाद नया पासपोर्ट अपडेटेड नाम या जीवनसाथी के विवरण के साथ स्पीड पोस्ट से आता है।
साथ ले जाने वाले दस्तावेज़
- री-इश्यू के लिए पुराना मूल पासपोर्ट और उसके पहले व आखिरी पन्ने की स्व-सत्यापित कॉपी।
- विवाह प्रमाणपत्र या Annexure J (दोनों पति-पत्नी के हस्ताक्षर वाली संयुक्त फोटो घोषणा)।
- जन्मतिथि का प्रमाण और फोटो पहचान पत्र (आधार, पैन, वोटर आईडी आदि)।
- आपके नाम का पते का प्रमाण, या पता वही हो तो जीवनसाथी के पासपोर्ट की कॉपी।
- लागू हो तो पहले जीवनसाथी का तलाक का आदेश या मृत्यु प्रमाणपत्र।
- एप्लीकेशन रसीद और अपॉइंटमेंट लेटर।
ज़रूरी बातें
- पहले आधार और पैन में नाम अपडेट करवा लें, या कम से कम अपने दस्तावेज़ एक जैसे रखें, ताकि पासपोर्ट से कोई मिसमैच न हो।
- नाम बदलने से पासपोर्ट नंबर पर असर नहीं पड़ता, लेकिन नई बुकलेट पुरानी की जगह लेती है और पुरानी रद्द होकर लौटाई जाती है।
- वीज़ा और हवाई टिकट पर वही नाम होना चाहिए जो पासपोर्ट पर छपा है। जल्दी यात्रा करनी हो तो पासपोर्ट पहले से अपडेट करवा लें।
- सिर्फ स्पेलिंग सुधार वाले बदलाव में नए सरनेम की तुलना में कम दस्तावेज़ लग सकते हैं।
